Enjoy the convenience of submitting ATA Member’s Annual Report by following this simple guide:

  1. Read page 193 (Annual Report) of ATA Accreditation Manual.
  2. Then prepare the ff. Spreadsheets (Excel files):
  • Educators in campus-based programs
  • Educators in non-campus-based programs
  • Extension/Study Centers for non-campus-based programs
  • Program Summary

  1. As soon as you are done with #1 and #2 above, you are now ready to fill-up the Annual Report Form.
  2. Go to this link: https://forms.gle/4LcxHZnXv92qMhLL6 and complete all the required information.
  3. Attach the four Excel files and submit.
  4. You will receive an email confirming that our office has received your documents.
